|  【】目的:评价苏茵解毒颗粒联合非奈利酮治疗糖尿病肾病(DKD)湿热瘀毒证的临床疗效与安全性。方法:选择2022年6月到2023年12月期间收治的96例Ⅲ-Ⅳ期DKD(湿热瘀毒证)患者,采用随机数分配方式,将患者均衡分为两个小组,各包含48名。两组患者均进行常规治疗,包括降糖、降血压及调整血脂等。在此基础上,对照组采用非奈利酮治疗,而观察组则在对照组的治疗方案中增加苏茵解毒颗粒,疗程均为6个月。比较两组患者治疗前后尿白蛋白与肌酐比率(UACR)的变化情况、估算肾小球滤过率(eGFR)、血肌酐(Scr)、尿素氮(BUN)、炎症因子(TNF-α、IL-6)水平变化、临床综合疗效及不良事件。结果:经过治疗后,两组病人的尿微量白蛋白与肌酐比率(UACR)、血清肌酐(Scr)、血尿素氮(BUN)及炎症因子含量均显著下降(P<0.05)。相较于对照组,观察组的UACR降低更为显著(P<0.05),血清中的肿瘤坏死因子-α(TNF-α)和白细胞介素-6(IL-6)水平更低(P<0.05),与此同时,观察组的估算肾小球滤过率(eGFR)较治疗前有所提升(P<0.05)。观察组的临床治疗有效率(89.58%)明显优于对照组(72.92%)(P<0.05)。两组患者不良反应发生率无差异(P>0.05),且均未出现严重不良事件。结论:在非奈利酮及常规治疗基础上,联合应用苏茵解毒颗粒能更有效地降低DKD患者的尿蛋白水平,减轻机体炎症状态,改善肾功能,且不增加不良反应风险,是治疗DKD湿热瘀毒证安全有效的方案。 |

| Objective To evaluate the clinical efficacy and safety of Suyin Jiedu Granules combined with finerenone in the treatment of diabetic kidney disease (DKD) with dampness?heat and blood stasis syndrome. Methods A total of 96 patients with stage III?IV DKD (dampness?heat and blood stasis syndrome) admitted from June 2022 to December 2023 were randomly divided into two groups, with 48 cases in each group. Both groups received conventional treatment including hypoglycemic, antihypertensive and lipid?regulating therapies. On this basis, the control group was treated with finerenone, while the observation group was treated with Suyin Jiedu Granules combined with finerenone. The treatment course was 6 months. The changes in urinary albumin?to?creatinine ratio (UACR), estimated glomerular filtration rate (eGFR), serum creatinine (Scr), blood urea nitrogen (BUN), inflammatory factors (TNF?α, IL?6), clinical comprehensive efficacy and adverse events were compared between the two groups before and after treatment. Results After treatment, UACR, Scr, BUN and inflammatory factor levels in both groups decreased significantly (P<0.05). Compared with the control group, the observation group showed a more significant reduction in UACR (P<0.05), lower levels of TNF?α and IL?6 (P<0.05), and a significant increase in eGFR compared with that before treatment (P<0.05). The clinical total effective rate in the observation group (89.58%) was significantly higher than that in the control group (72.92%) (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05), and no severe adverse events occurred. Conclusion On the basis of finerenone and conventional treatment, the addition of Suyin Jiedu Granules can more effectively reduce urinary protein levels, alleviate systemic inflammation, and improve renal function in patients with DKD, without increasing the risk of adverse reactions. It is a safe and effective therapeutic option for DKD with dampness?heat and blood stasis syndrome. |
