Exploring the Clinical Diagnosis and Treatment of Phlegm-Dampness Obstruction-Type Polycystic Ovary Syndrome Based on the San Jiao Theory

      Polycystic ovary syndrome (PCOS)is a common endocrine and metabolic disorder among women of childbearing age. Its etiology is complex and diverse, among which the phlegm-dampness stagnation syndrome is a common syndrome type. Based on the theory of the San Jiao in traditional Chinese medicine, this study deeply analyzes its pathogenesis: the lung in the upper jiao fails to disperse and descend, leading to the obstruction of the water passages; the spleen in the middle jiao fails to transform and transport, resulting in the accumulation of dampness into phlegm; the kidney in the lower jiao is deficient in qi, causing insufficient the heavenly essence and impaired qi transformation. The dysfunction of these three viscera together leads to the phlegm-dampness stagnation in the uterus and Chong and Ren meridians. Based on the Sanjiao theory, this study proposes a “treatment of the three jiao together” strategy: for the upper jiao, the method of dispersing lung and resolving phlegm is adopted, using herbs such as Platycodon grandiflorus and Mori Cortex to unblock the lung qi and water passages; for the middle jiao, the key is to strengthen the spleen and remove dampness, using Atractylodes macrocephala and Poria to invigorate the spleen and transform phlegm; for the lower jiao, the focus is on warming the kidney and assisting yang, using Eucommia ulmoides and Epimedium to warm the kidney yang and promote qi transformation and water metabolism. Clinical practice has shown that this therapy can effectively regulate the endocrine and metabolic functions of patients, improve symptoms such as menstrual disorders, obesity, and facial acne, and promote follicular development, providing new ideas and effective solutions for the treatment of phlegm-dampness stagnation syndrome in PCOS
