      Polycystic ovary syndrome (PCOS) is a common endocrine and metabolic disorder that affects female fertility during the reproductive years. Obesity frequently coexists with PCOS and exacerbates metabolic dysregulation. In traditional Chinese medicine (TCM), obese PCOS falls under the category of “Obesity related infertility”. Classic texts such as Fuqingzhu Nvke and Yifang Jijie attribute this condition to phlegm-dampness and advocate strengthening the spleen and resolving dampness as effective therapeutic principles. Grounded in the theories that “the kidney governs reproduction” and “spleen deficiency impairs the kidney”, treatment for infertility should incorporate kidney supplementation; thus, the approach of warming the kidney and strengthening the spleen is indicated. Wenjian Qigong Decoction (WJQG) was formulated based on this method. Through clinical case analysis, this paper discusses the etiology and pathogenesis of obesity (phlegm-dampness) type PCOS and explores the clinical application experience of WJQG, aiming to provide references for the TCM diagnosis and treatment of this disease.
