      The pathological basis of osteoporosis (OP) involves not merely impaired bone formation or excessive bone resorption, but also the combined effects of multiple factors, including endocrine dysfunction, immune dysregulation, and an imbalance between osteogenic and adipogenic differentiation of bone marrow mesenchymal stem cells (BMSCs), ultimately leading to disrupted intercellular communication within the bone metabolic microenvironment. Total flavonoids from Drynariae Rhizoma constitute an important bioactive fraction responsible for its bone-protective effects, with naringin and naringenin being representative constituents. Current evidence suggests that total flavonoids from Drynariae Rhizoma and their representative constituents may improve the bone metabolic microenvironment in OP by regulating estrogen receptor-related signaling and correcting the shift of BMSCs from osteogenic toward adipogenic differentiation. Previous studies have mainly discussed their anti-osteoporotic mechanisms from the perspective of molecular signaling pathways. Therefore, this review summarizes the relevant evidence from the perspective of the bone metabolic microenvironment, aiming to provide a reference for further investigation and clinical application of total flavonoids from Drynariae Rhizoma in the prevention and treatment of OP.
