En Abstract:
Diabetic kidney disease (DKD) is a common microvascular complication of diabetes. Its pathogenesis is complex, and current therapies cannot completely halt the progressive decline in kidney function. In traditional Chinese medicine (TCM), DKD falls within the category of Xiaoke kidney disease and is characterized by root deficiency and branch excess, with deficiency and excess occurring simultaneously. Huanglian Pill, composed of Coptidis Rhizoma and Rehmanniae Radix, combines heat-clearing with yin-nourishing effects and corresponds to the pathogenesis of yin-fluid depletion, internal accumulation of heat, and intermingled dampness and blood stasis in DKD. Modern studies suggest that Huanglian Pill and its related constituents may exert renoprotective effects by regulating glucose and lipid metabolism and renal energy metabolism, attenuating inflammation and oxidative stress, preserving the glomerular filtration barrier, and inhibiting renal tubular injury and fibrosis. This article reviews the TCM theoretical basis, modern pharmacological effects, and potential mechanisms of Huanglian Pill, aiming to provide a scientific basis for its use in the treatment of DKD.
